Imprinting on All W-2 and 1099 Forms

Add $27.80 per 1,000 forms or any fraction of 1,000. Imprinting includes the employer's name, address and I.D. number(s). Each 1099 form is considered a separate imprint for price computation, even when both 1099 and W-2 forms are to be imprinted the same. Each change of copy (name and address) is considered a separate imprint for price computation. Be sure to provide payer's phone # for 1099 forms.

State Name Imprinting on W-2 Forms

If the state name and employer's information is to be printed, the imprint charge is $28.70 per 1,000 forms or any fraction of 1,000. If only the state name is to be printed on the forms, the imprint charge is $27.80 per 1,000 forms or any fraction of 1,000. No state names will be printed unless the order so specifies.
