Double Window Envelope to be used on our W-2 4-per-page Laser Printer Forms (items # L4UP, BL4UP, L4UPR & BL4UPR). All envelopes are tinted for privacy and have the “tax information” notation.

Envelope size: 5 5/8″ x 9″

Top Window:
Size: 3/4″ x 3 7/8″
Position: 1/4″ from left, 3 1/2″ from bottom

Bottom Window:
Size: 3/4″ x 3 7/8″
Position: 4 3/8″ from left, 2 1/2″ from bottom

Use envelope DW4S for 4-squares-per-page employee copies printed with most software programs including QUICK & EASY. Some software programs print the employee copies 4-per-page as a slightly different layout requiring the DW4MW envelope. Please refer to the dimensions shown to verify the window alignment or you may request a sample.
